Connecting to SQL using software Querious

Hi, I am trying to connect to a SQL database using Mac OS X software Querious and because I am a newbie I am wondering what to put in the following fields. So far unsuccessful..

Here is what they are looking for

Host:
user:
password:
database:
socket:
port: 3306
Security SSH | SSL

I have tried several different seranios anybody out there knows the correct way to do this. Any help will be appreciated.
